چکیده

W eight loss greater than 6% independently correlates with reduced survival in patients with congestive heart failure (CHF). Cardiac cachexia is a gradual and graded process, with wasting affecting muscle, bone, adipose tissue and the heart. Reduced appetite has been proposed as a important factor for weight loss caused by cardiac cachexia. The mechanism of cardiac cachexia remains largely unclear. Gastric myoneural inhibition and gastrointestinal hypomotility with delayed gastric emptying is commonly observed, and gut hormones may also play a role, because the anorexia is often characterised by a premature feeling of fullness and loss of hunger. The gut hormone ghrelin stimulates hunger, increases food consumption. and was previously shown to be raised in patients with cardiac cachexia and CHF. Ghrelin is produced from the stomach and usually rises in anticipation of a meal and then falls after the ingestion of nutrients. The gut hormone PYY3–36, acting as a terminator of hunger, is produced by the L cells in the distal gastrointestinal tract; it reduces appetite and 24 hour food intake by binding to the neuropeptide Y Y2 receptor in the hypothalamic arcuate nucleus. Severe pulmonary hypertension results in a low cardiac output and neurohumoral activation. Patients frequently report loss of appetite and weight, and since it occurs in a young population they have little co-morbid disease, which itself might affect appetite. This study aimed to evaluate dynamic PYY and ghrelin responses to a standard meal in control subjects and patients with cardiac cachexia associated with pulmonary hypertension.

Thalassemia Associated Pulmonary Hypertension

Cardiac disease is the main cause of death in both forms of thalassemia; thalassemia major (TM) and thalassemia intermedia (TI). Pulmonary hypertension (PH) is one of the cardiopulmonary morbidities with high mortality that, if not treated, may trigger right-sided heart failure and premature death. PH is defined as a mean pulmonary artery pressure of ≥25 mmHg at rest or ≥30 mmHg during exercise...

Gut Hormones, Appetite Suppression and Cachexia in Patients with Pulmonary TB

BACKGROUND Cachexia is a hallmark of pulmonary tuberculosis and is associated with poor prognosis. A better understanding of the mechanisms behind such weight loss could reveal targets for therapeutic intervention. The role of appetite-regulatory hormones in tuberculosis is unknown. METHODS AND FINDINGS 41 subjects with newly-diagnosed pulmonary TB (cases) were compared to 82 healthy controls...

A case of severe pulmonary hypertension associated with common arterial trunk and VSD in a 9 years old child

Background: Common arterial trunk (CAT) is a rare congenital heart disease, and often leads to the early development of pulmonary hypertension and disability. Among the critical structural heart defects, the frequency of CAT is 3%, which reflects the severe hemodynamic disturbances. The natural course of the disease is characterized by a high mortality rate up to 88% during the first year of li...

Evaluation of Pulmonary Hypertension In First Degree Relatives Of Patients With Primary Pulmonary Hypertension

Background: Pulmonary hypertension (PH) was defined for the first time in 1951 as primary pulmonary hypertension (PPH). Some studies emphasized on the role of genetics in the development of pulmonary hypertension in family members of affected patients. So, in this study we evaluated the prevalence of pulmonary hypertension in first degree family of patients with documented PPH. Methods: In thi...
